Conditional acceptance may be granted based on an interim transcript.

Application Checklist:

• Answer all questions on the application form.
• Sign the application form.
• Complete your autobiographical sketch.
• Arrange for all official transcripts to be sent.
• Distribute reference forms.
• Enclose application fee. 
• Enclose a picture of yourself.
