Application Procedures

Application guidelines and forms. Conditional acceptance may be granted based on an interim transcript.

Application Checklist:
(Applies to full application only)

  • Answer all questions on the application form.
  • Sign the application form.
  • Attach a recent picture of yourself. Write your name on the back for identification in case it is separated from the application.
  • Complete your autobiographical sketch.
  • Arrange for all official transcripts to be sent from former undergraduate/graduate work.
  • Distribute three letters of reference indicating background, character, personal maturity, motivation, health, education and capability for study at the appropriate level.
  • Enclose application fee. 
  • Sign the Community Life Commitment form.
